Social Prescribing and Community Navigation
NHS Kent and Medway want your views about Social Prescribing or Community Navigation Services
Social Prescribing and Community Navigation is a service that can empower you to make a positive change in your life and within your community. It can link you to support for a range of different challenges that matter to you, including financial support, housing, information and advice, carers support and social connections and activities to help you live a happy, independent and safe life.
Link Workers and Community Navigators have time to build trusting relationships, start with what matters to the person, create a shared plan and introduce people to community support.
These services can especially help people who;
- have one or more long term condition
- need support with their mental health
- are lonely or isolated
- have complex social needs which affect their wellbeing.
It helps people to get more control over their healthcare, to manage their needs and in a way that suits them.
NHS Kent and Medway are now writing a strategy that will improve social prescribing and community navigation across the county.
If you have had support from a community navigator or link worker they would like to know about your experience of the service, and what suggestions you have for the development of social prescribing in the future.
NHS Kent and Medway need your views and opinions about the services, which will help them write the strategy and continue to build a better service for communities.
This survey is for those who deliver social prescribing or community navigation. If you are professionally trained as a Social Prescriber, Link Worker or Community Navigator please tell NHS Kent and Medway your views.
Please tell them what works, and what could be improved for you to help you carry out your role. This survey will close on Monday 20th March.
