Imago is a well-respected charity and social business with 60+ years’ experience.

We deliver commissioned services and social action with vulnerable adults, children, young people, carers and the cared-for across Kent, Medway, East Sussex and London. We currently support over 39,000 people across our services.

Our extensive services are delivered in partnership with both local authorities and the NHS, supported by financial and in-kind support from charitable trusts, the corporate sector and community fundraising.

What can Imago social prescribers do for your PCN?

Social prescribing is a key component of universal personalised care and helps bridge the gap between primary care and the local community support networks. Imago has been delivering high quality social prescribing services for over 9 years across a diverse range of communities and care networks.

We have a range of service models that can be tailored to suit your needs including:

  • Fully staffed and managed service 
  • Support services - training and induction, supervision and support, CPD
  • Project support – drop-in sessions, clinics, digital inclusion, workforce development

Social prescribing for children and young people in East Sussex.

This short video captures the impact of a pilot project using social prescribing and positive activities to support children’s mental health and emotional wellbeing in East Sussex. The project was delivered by Imago, managed by East Sussex County Council, and funded by NHS Sussex. The four schools involved were Parkside Primary, Ringmer Primary, South Malling Primary and Wadhurst Primary. The video was created by Make Good Trouble.
