If you are an adult aged 55 and over, or aged 18 and over with complex needs, our community navigators can provide you with a range of one-to-one support. We help make a difference to your health, wellbeing, confidence, independence and motivation.

Our free Navigation services offer practical advice, emotional support, and one-to-one guidance tailored to your needs. We’ll help you access community, health, and social care services while building your wellbeing, resilience, and confidence to live independently.

What support will I receive?

Our navigators will guide you, and your family and carers, through accessing essential services and local support, enabling you to find the right opportunities and make your own choices.

helping you manage money and claim any income and benefits you may be entitled to.

arranging mobility adaptations, equipment aids and assistive technology to support you to live safely and independently in your home.

helping you to complete a wide range of assessment and application forms.

The Hub offers information and advice for carers and professionals. As your first point of contact when you join the service, our friendly team will make sure they understand your needs and guide you through support available in your area. The Hub is available for you to call with any queries Monday–Friday 9am-5pm.

We can visit you at home to meet you and your family, to work out what you need, help you with your immediate issues and agree a plan for what happens next.

Complete and submit the online referral form or contact our central Hub 0300 011 1965. Our phone lines are open from Monday–Friday, 9am-5pm. You can also email us navigation@imago.community.
