Supporting individuals and their families and carers to navigate through health, social care and voluntary support services.

Our wellbeing navigation service is free, offering impartial information, advice and guidance to adults over the age of 18. We’ll help you access community, health, and social care services and work with you, your family and carers to help you find the right local support and opportunities to improve your wellbeing.

If you are a patient at Medway Maritime Hospital, our hospital navigator can provide you with practical support to ensure that your home is safe, and care is in place so that you can be discharged when you are ready.

We also offer a specialist service to support adults aged 18-45 with advice on housing, homelessness, education, employment and benefits.

Medway Maritime Hospital patients were supported to return home

I have had a stroke, so communicating is an issue, the navigator was understanding and patient. She listened to me and gave me time to think and speak.  I really appreciate the help with the forms, it was very kind.
