Our wellbeing navigation service is free, offering impartial information, advice and guidance to adults over the age of 18. We’ll help you access community, health, and social care services and work with you, your family and carers to help you find the right local support and opportunities to improve your wellbeing.
If you are a patient at Medway Maritime Hospital, our hospital navigator can provide you with practical support to ensure that your home is safe, and care is in place so that you can be discharged when you are ready.
We also offer a specialist service to support adults aged 18-45 with advice on housing, homelessness, education, employment and benefits.

We can provide information and guidance, practical assistance and emotional support, and help to access a range of community, health and social care support services.

Access to financial support and advice
helping you manage money and claim any income and benefits you may be entitled to.
Maintaining and adapting your home
arranging mobility adaptations, equipment aids and assistive technology to support you to live safely and independently in your home.
Applying for other services, grants and support
helping you to complete a wide range of assessment and application forms.
Building your social networks
exploring peer support groups, befriending and creative activities, to make social connections and develop your interests.
Accessing other support
helping you find support for a range of needs including domiciliary care, housing, transport, gardening, handyman services, shopping etc.
Imago Hub
The Hub offers information and advice for carers and professionals. As your first point of contact when you join the service, our friendly team will make sure they understand your needs and guide you through support available in your area. The Hub is available for you to call with any queries Monday–Friday 9am-5pm.
Meeting you at home
We can visit you at home to meet you and your family, to work out what you need, help you with your immediate issues and agree a plan for what happens next.
Supporting you when you leave hospital
If you are in hospital, we can make sure the right support is in place at home, so that you can be discharged safely when medical staff say you are ready.
