GP Referrals
Carers who provide high levels of care are more than twice as likely to suffer from poor health.
Approximately 12% of the patients who are registered at your GP or Medical Practice could be looking after someone at home. Enabling carers to access specialist support can prevent them from becoming high intensity users of health services.
Our Community Care Navigators assess the needs of the Carer and liaise with other services, agencies and professionals.
Referring a client or patient is simple

Access to services
We can refer to services such as
- Replacement care
- Nursing care at home
- Day care centre placements
- Domiciliary care (anything from making sure someone takes their medication, getting someone up and dressed in the morning, to 24-hour care)
- Residential or nursing home placements
- Domestic help (general house work duties including ironing and cleaning services or someone to do the shopping).
Most services will be met with funding but, if the only way forward is through a place in residential or live-in care, we can access additional grant funding specifically for that service (funded by the local Clinical Commissioning Groups).
